Spouse & Caregiver Scholarship Quick Intake

This form is exclusive to spouses and caregivers who have been connected to HOPE's Military Spouse & Caregiver program by completing a full Client Intake within the last six months.

You will receive the link to the Spouse & Caregiver Scholarship application via email within 48 business hours after the application period opens.

Fall: March 1 - April 30

Spring: August 1 - September 30

*Closing dates may vary and will be indicated on the scholarship application
